Physiotherapy Under a Medicare Care Plan

If you live with a chronic condition — like back pain, arthritis, diabetes, or a heart or lung condition — your GP can refer you for physiotherapy or exercise physiology under a Medicare chronic condition management plan (previously known as an EPC or CDM plan).

The plan gives you up to five Medicare-rebated allied health visits each calendar year, shared across the providers your GP refers you to. We process your Medicare rebate on the spot at each visit, so you only pay the small gap.

How Funding Works

How Medicare Covers Physiotherapy

Medicare doesn’t cover standard physiotherapy visits — but it does subsidise treatment for people with chronic conditions through a GP chronic condition management plan (the scheme formerly called an Enhanced Primary Care (EPC) plan or Chronic Disease Management (CDM) plan).

Your GP assesses your eligibility, creates the plan, and refers you to us. Each eligible visit attracts a Medicare rebate, which we process for you on the spot at the clinic — you only pay the difference between our fee and the rebate.

Ask your GP whether a chronic condition management plan is right for you, and request a referral to Activ Therapy. Already have one? Go straight to step 2.

Book With Your Referral

Call us or enquire below and mention you have a Medicare care plan. Bring your referral and Medicare card to your first visit.
